This distinction is important in Trump's case because the abnormalities I see are not confined to forgetting names. They appear across several cognitive systems.
The Limitations of Available Evidence
I do not have access to Donald Trump's brain MRI, and I have not personally administered a complete formal neuropsychological test battery to him. Those limitations are important if the objective is to determine the precise neurological cause of deterioration or quantify individual deficits. Determining that cognitive deterioration exists, however, and determining the neuropathological process responsible for it are two different questions.
Trump's condition has deteriorated to the point that, in my clinical judgment, the publicly available behavioral record is sufficient for me to identify a significant pattern of cognitive decline. An MRI could help determine the etiology, and formal neuropsychological testing could quantify individual deficits much more precisely. Neither change what can already be observed repeatedly in his public behavior.
The amount of longitudinal behavioral material available is extraordinary. Trump speaks publicly for hours. He answers questions, gives interviews, holds press conferences, participates in meetings, and has generated thousands of hours of recorded behavior over many years. This provides an unusual opportunity to compare his present functioning with his own earlier functioning.
The Famous 30 Out of 30
Trump repeatedly points to his perfect score on the Montreal Cognitive Assessment, or MoCA, as evidence of his cognitive health and sometimes presents it almost as evidence of exceptional intelligence. A score of 30 out of 30 is certainly a favorable screening result, but the MoCA is a brief cognitive screening instrument. It is not a comprehensive neuropsychological evaluation.
The MoCA samples cognitive functions to screen for impairment. It does not substitute for an extensive neuropsychological battery designed to measure individual abilities in depth. A perfect screening score therefore does not establish that every cognitive system continues to function at the individual's previous level.
The clinically relevant question is not whether Trump could perform successfully on a brief screening instrument at a particular moment. It is whether his cognitive functioning has changed significantly from his own previous baseline. His longitudinal public behavior provides considerable information with which to examine that question.
Executive Functions
Executive functions are the brain's supervisory system. They allow us to establish goals, plan, distinguish relevant information from irrelevant information, inhibit impulses, anticipate consequences, maintain information in working memory, change strategies when circumstances change, recognize mistakes, and monitor our own behavior.
One well-documented example is the increasingly digressive organization of Trump's public discourse. A 2024 Washington Post review of dozens of his speeches, interviews, and other appearances found that his speeches had become longer and more repetitive than in previous campaigns and that he was shifting more abruptly among subjects and between scripted material and improvisation. Trump himself calls this speaking style "the weave." He argues that the ability to leave a subject and eventually return to it demonstrates extraordinary memory. From a neuropsychological perspective, however, another question must be asked: how effectively can the individual establish a cognitive target, maintain it, suppress irrelevant information, and efficiently return to the original task?
A second example is his difficulty inhibiting irrelevant material once it enters his train of thought. During public appearances, relatively straightforward questions can produce lengthy responses that move through unrelated subjects, personal grievances, political opponents, memories, and peripheral details. The clinical issue is not whether Trump can eventually return to the original subject. It is the amount of unrelated cognitive material that intervenes between the question and an organized response.
Trump has always been impulsive, verbose, and prone to digression. That is precisely why comparison with his previous baseline is important. What concerns me is the increased frequency, intensity, and disorganization of the behavior. A healthy executive system does not merely generate behavior. It regulates behavior. Put simply, the brain has an accelerator, but it must also have brakes.
Memory
Memory is not simply remembering someone's name. Information must first be encoded, then retained, retrieved when needed, and incorporated into existing knowledge. The pattern of memory failures and the response to them are therefore more informative than an occasional forgotten word.
